What is Care Management?
The Care Management Program is a Consumer focused collaborative process which assesses individual's needs and plans, implements, coordinates, monitors, and evaluates options and services, using formal and informal resources, to meet home bound frail older adults and their caregivers' needs. This program is under the New Jersey ADRC system.
A care manager is someone who is a registered nurse, social worker and/or other health related professionals who is trained by the New Jersey ADRC system. A care manager is responsible for care planning, monitoring of care needs, service delivery, and assisting with various programs and services to help individuals to remain in the community.

Funding Acknowledgement
This program is funded in party by the Bergen County Division of Senior Services through the Older Americans Act and other funding sources. All services are available at no charge to eligible Bergen County seniors.
