Jersey Assistance for Community Caregivers (JACC)
The Jersey Assistance for Community Caregivers (JACC) program is a state-funded initiative that provides a comprehensive range of home and community-based services to individuals who meet the clinical criteria for nursing home-level care but wish to continue receiving care in their own homes within their community. JACC encompasses a variety of services designed to complement the support provided by a network of caregivers, including family members, friends, and neighbors.
a. Services Provided by JACC:
Due to funding limitations, it is generally not possible to receive duplicate services (i.e., receiving more than one of the same type of service simultaneously).

b. Eligibility Criteria

Must be a U.S. citizen or permanent resident aged 60 or older, residing in New Jersey (and not currently residing in a care facility).

Must meet the clinical criteria for nursing home-level care.
c. Program Income Limits

$4,855/month (Individual), $6,582/month (Couple)

Resources: $40,000 or less (Individual), $60,000 or less (Couple)
