Long Term Medicaid (MLTSS)
MLTSS is a Medicaid program designed for low-income individuals. It is a New Jersey state program intended for elderly patients or people with disabilities who require nursing home care due to illness but wish to remain in their own homes. Regardless of whether or not one currently possesses Medicaid coverage, this program offers a comprehensive range of services designed to meet both the medical and non-medical needs of individuals with chronic illnesses, disabilities, or advanced age.

a. Eligibility Requirements
Residents of Bergen County who are aged 65 or older, or individuals aged 18 or older with a disability.
Permanent Residents (holding status for at least 5 years) or U.S. Citizens.

This program applies to individuals diagnosed by a physician with a chronic condition (defined as a condition that impairs the ability to perform daily activities such as eating, bathing, or moving independently), making it difficult for them to manage daily life on their own.

b. MLTSS Program Income Requirements (As of 2026)
Individual Income: $2,982 or less per month.
Assets: Bank balance of $2,000 or less (applies to both individuals and couples).

c. Services Provided through MLTSS (Services may vary depending on the insurance provider):
Home Care – Nurses or home health aides visit the home to provide care for the patient.
Medical Equipment Assistance (Canes, walkers, wheelchairs).
Meal Delivery (Once daily on weekdays, or once per week).
Support Programs for Caregivers (Family members).
Medical Adult Daycare.
Nursing Home Services.
